The 2024 revisions of the McDonald criteria involve major changes with the aim of increasing sensitivity and specificity for making an earlier and accurate diagnosis of MS. Part 2 applies the revised criteria in people presenting with typical symptoms for a relapsing or progressive course, those with non-specific symptoms and Radiologically Isolated Syndrome (RIS), and in specific patient populations (including pediatric, older, and those with co-morbidities). The typical, Green Flag MRI appearance suggestive of MS lesions is contrasted with the warning of Red Flag changes. Challenges in applying the criteria — including misdiagnosis, overlapping syndromes, and global applicability — are then discussed. Presentations are supported by interactive case discussions.
