Spinal cord disease is important in most people with MS , but is less emphasized in patient care, basic, clinical research, and therapeutic trials. Clinically relevant spinal cord disease in MS is crucial to recognize. How and when to image the spinal cord with MRI in people with MS is an important clinical concern. Progressive spinal cord atrophy and ‘silent’ multiple sclerosis progression is emerging as an important multiple sclerosis prognostic biomarker. Given this, the importance of MS spinal cord disease including clinical presentations and neuroimaging will be discussed.
